Food Services
- Congregate Meals - Senior meals provided in a group setting, Monday through Friday at noontime.
- Emergency Meals - Access to food pantry, usually due to unforeseen financial and health situations.
- Food Assistance - The Food and Nutrition Program is designed to promote the general welfare and to safeguard the health and well being of the nation's population by raising the levels of nutrition among low-income households.
- Food Pantry - Connecting those in need with non-perishable foods.
- Home Delivered Meals - Meals delivered to every corner of Chatham and Orange County by dedicated volunteers.
Home Care
- At Home Assistance - In-home assistance with daily living and transportation.
- Hospice Aide Visits - Assistance with personal care activities of daily living provided in home setting, as well as homemaker chores necessary to maintain individual in a safe and sanitary environment.
- Special Assistance to Adults - Special Assistance-In Home program provides an alternative to placement in an Adult Care Home for individuals who could live at home safely and with additional support services.
- Specially Trained Hospice Volunteers - Offering a wide range of services to hospice families, such as companionship and supportive listening, respite for caregivers, light housekeeping and shopping. Trained bereavement volunteers, licensed massage therapists and musicians to serve hospice families.
Housing
- Continuing Care Retirement Community - CCRCs are an alternative housing option that offers a full continuum of care, ranging from fully independent units, to assistance with personal care in assisted living apartments, to long-term care in a skilled nursing facility.
- Energy Assistance - Providing eligible low-income families assistance with heating bills.
- Fan Distribution Program - Fans made available to seniors and persons with disabilities on a seasonal basis to help alleviate summertime heat.
- Home Repair - Supporting independence in the home by providing materials and labor to improve safety of homes. Wheelchair ramps, grab bars and transition chairs.
- Home Safety Evaluations - Providing home safety assessments and recommendations for home modifications.
- Housing - Assistance with locating adult care homes.
- Placement Services - Helping aging or disabled adults find appropriate living and healthcare arrangements when their health, safety and well-being can no longer be maintained at home.

- Medicaid Questions - Assisting those with questions regarding, eligibility, cost, application information and programs. Medicaid is a program operated through governmental funds to assist eligible individuals and families with low income, resources and/or certain disabilities.
- Medicare Questions - Assistance with obtaining information and answering questions concerning Medicare Part A and Part B coverage and claims, durable medical equipment, and Medicare Part D Prescription Drug Plans. Medicare is a senior health insurance program. Seniors’ Health Insurance Information Program (SHIIP) counsels individuals on health care plans and options.
Medical Care
- Adult Day Care - A day program, which supports personal independence and promotes their well-being. Services are provided in a certified home or center.
- Advance Care Planning -
- Chronic Disease Case Management for Children & Adults - Case managers work directly with patients to assist in complying with plans established by the primary care provider. Case managers conduct home visits and assist patients with everything from medical appointments and medical education to locating transportation and referral to community based resources. Service intended for Medicaid recipients only.
- Emergency Room - Emergency and urgent care facilities in Chapel Hill provide around-the-clock services for accident and trauma victims and for people with minor, but urgent medical needs.
- Health Advice (Health Link Nurse Line) - HealthLink provides quick, easy access to the resources and expertise of the doctors and staff, free of charge. HealthLink is available 24hours/day to answer your questions.
- Heart Failure Case Management Program - Program focuses on promoting self management and adherence in high risk heart failure patients and working with clinicians to follow evidence based clinical practice guidelines. Self management education available to patients and caregivers. Service intended for Medicaid recipients only.
- Hospice Aide Visits - Assistance with personal care activities of daily living provided in home setting, as well as homemaker chores necessary to maintain individual in a safe and sanitary environment.
- Hospital - Information pertaining to the multiple services offered by the UNC-Chapel Hill Hospital system.
- Hospital to Home Transitions - Providing care coordination to Medicaid patients being discharged from the hospital.
- Intermittent Skilled Nursing Visits - Access to trained RNs experienced in providing care outside the hospital or nursing home. Nursing staff are available 24 hours a day for emergency on-call coverage.
- Long-Term Care Planning - Assistance with long-range planning, relocation, home safety modifications, health insurance questions, care planning decisions, and counseling regarding age-related and care giving issues.
- Medical Social Workers - Emotional support provided to hospice families. Short-term counseling, referral to and coordination with community resources and assistance with living arrangements, finances, long-range planning and advanced care directives.
- Medical Supplies and Equipment - Access and information pertaining to medication dosages, medical supplies and equipment related to the terminal illness.
- Medication Management - Educate patients and providers to ensure patients are taking the appropriate type, dosage and combination of medications.
- Out Patient Clinics - Allows patients to obtain care in a convenient, outpatient clinic environment and return to their normal routine as quickly as possible. Multiple locations throughout North Carolina to provide highly skilled care in state-of-the-art facilities.
- Patient Advocacy -
- Physical Therapy - Physical therapy services as well as exercises and therapy services related to preventing falls.
- Respite Care - Short-term, temporary relief for caregivers of older adults and persons with disabilities.
- Short-Term Care Planning - Obtain information, counseling and services to help navigate the medical, personal and social transitions that may accompany aging.
Mental Health, Developmental Disabilities & Substance Abuse Services
- Community Resource Court - Addresses the treatment needs of people with mental health issues who have become involved with criminal courts. Program links offenders with services and supports that help manage mental illness.
- Mental Health Services - Access to Person Centered Planning to help develop a plan of support, service(s) and treatment for mental health issues facing an individual or family member.
- Substance Abuse Services - Access to Person Centered Planning to help develop a plan of support, service(s) and treatment for substance abuse issues facing an individual or family member.
- Treatment Alternatives for Safer Communities (TASC) - Provides quality services and accessibility for drug-involved and mentally ill offenders. TASC combines the influence of legal sanctions with recommended levels of care to permanently interrupt the vicious cycle of addiction and criminality.

- Dietary Counseling - Counseling services provided by licensed dieticians to ensure patients are knowledgeable of dietary recommendations and limitations.
- Nutritional Supplements - Assistance for specific nutritional supplements for a variety of needs.
- Wellness - Various programs, services, workshops, and fitness classes designed to help older adults maintain their well-being.
Programs
- Adult Protective Services - Available to adults who are incapacitated due to physical or mental disability and who are in need of protection because of abuse, neglect or exploitation.
- Eldercare Mediation -
- Emergency Assistance Program - Financial assistance, energy assistance, housing assistance.
- Emergency Shelter Services Operations - Connecting the community to emergency shelters serving, primarily, older adults and disabled persons and, secondarily, the general population.
- Long Term Care Ombudsman Program - Ombudsmen investigate complaints about the care of residents in long-term care facilities and serve as residents' advocates. They provide mediation and education in an effort to resolve grievances and increase understanding of long-term care residents' rights.
- OPC STAR - Call center provides screening, triage and referral for consumers seeking mental health, substance abuse or developmental disability services.
- Public Benefits Counseling - Counseling services provided to answer questions and/or determine eligibility regarding Medicare, Medicaid and attaining equal access to all federal, state and local services that are available to the public.
- Senior Health Insurance Information Program - Program designed to offer expert and unbiased information regarding Medicare and insurance programs marketed to older adults.
- Veteran’s Services - Various programs and assistance for veterans in obtaining, retaining, or reinstating compensation, pension, medical and other applicable benefits and services.
Services for Persons with Disabilities
- Audio Information Service - Audio information service to disseminate current news, information and entertainment from the print media to people who can no longer read standard print.
- Blind Services - Services to help visually impaired individuals integrate smoothly into their environment and community. Eye certification, adjustment and self-support, orientation and mobility, and counseling.
- Deaf Services - Advocacy and consultation, communication support, information and referral, workshops and training.
- Deaf-Blind Services - Advocacy and consultation, communication support, information and referral, workshops and training.
- Disability Services - Providing assistance in locating the various services available in the community to persons with disabilities.
- Hard of Hearing Services - Advocacy and consultation, communication support, information and referral, workshops, training and outreach.
- Individual and Systems Advocacy - Advocacy provided to persons with disabilities to ensure equal access to all infrastructure, employment opportunities, fair housing or any other situation involving civil rights and equal opportunities.
Social
- Activities - Wellness programs and health education, social and recreational clubs and classes for seniors.
- Community Inclusion - Assisting persons with disabilities to become productive, full participants in society with equal access to community facilities.
- Trips - Various trips throughout the N.C area, including some overnight stays.
Support Groups & Training
- Adult Basic Education - Comprehensive educational program open to adults or teens, age 16 and older, with less than an eighth grade education. Persons may enroll in the ABE program to improve reading, writing, speaking, math, and critical thinking skills, as well as, helping children with homework, obtaining a better job or promotion and how to apply to college.
- Caregiver Support Group - Resources for caregivers, links for care management plans.
- Chaplain/Bereavement Services - Bereavement services for individual and family counseling and spiritual support, support groups, and assistance with funeral arrangements.
- Conflict/Communication Coaching -
- Counseling - Long-term and short-term planning, respite for family caregivers.
- Dementia Education - Dementia education for faith and civic groups and families. Caregiver support groups.
- Diabetic Support Group - Disease management education through monthly meetings and support groups.
- Grandparents Raising Grandchildren - Intergenerational caregiver programs designed to reach out to grandparents raising grandchildren.
- Independent Living Skills Training - Assisting people with disabilities to live independently and become productive, full participants in society.
- Mild Cognitive Impairment Support Group - Support groups available to residents 60 years and older and their families regardless of income.
- Peer Mentoring and Support - Peer to peer support provided to individuals with disabilities to help develop life skills. Support groups for various disabilities. Community room available as a meeting space for support groups. Please contact the center for more information regarding the community room.
- Respite -
- Speakers & Workshops - Education and training for caregivers and families.
- Telephone Reassurance - Telephone calls are made Monday - Friday to check on seniors' well being. Call for an in-home assessment prior to beginning telephone reassurance.
Transportation
- Medical Transportation - Assistance with non-emergency medical transportation, including appointments to Chatham County, the Chapel Hill area, and others.
- Transportation - Rural transportation funded by the State of North Carolina and Chatham County. Riders can take advantage of regularly schedule stops and return trips at subsidized rates.
